New Admission Frequently Asked Questions

  • You will be notified of the daily rate at the time of admission and you will sign an admission contract. There is a list of other items that are charged when used. This list will be provided to you at admission. Statements are mailed at the first of the month to pay in advance for the charges in the upcoming month. Your pharmacy and physician charge and bill separately for their services.

  • We will need to make copies of the any of the following cards if applicable: Medicare, Medical Supplement Card, Pharmacy Card, Medicare Part D, Social Security, and Medicaid. We also need a copy of any legal documents such as Powers of Attorney (Durable and or General Financial) and any Advance Directives such as a Living Will or IPOST.

  • Except for persons who receive their medications through the Veteran’s Administration, all prescription medications administered at the facility must be dispensed in a unit dose system to prevent the likelihood of errors. The facility will work with you in using up your home medication for a limited amount of time. All medications, prescription and over-the-counter, require a physician order in the nursing facility setting. All medications brought to the facility need to be given to nursing management to determine the appropriate way to dispense. At times, seniors who wish to administer their own medications may do so if this a planned intervention that follows quality guidelines and is approved by the physician.

  • Each unit is furnished with a bed and pressure-reduction mattress, nightstand, closet space, dresser space, and a chair if necessary. You are encouraged to bring in a chair or other small personal items such as pictures to make your unit more homelike. Items that cannot be allowed for safety reasons include; throw rugs, extension cords, door stops, heating pads/electric blankets, aerosol cans, electrical cheater plugs. All fans should be examined by maintenance to assure they are safe.

  • Phone service to units is a private arrangement between the telephone company and the resident/tenant’s family member. You might wish to consider a cell phone. The facility has phone/cell phones available for your use at any time.

  • Each room can have a television set. Hook-ups to antenna are available. Cable service is a private arrangement between the cable company and the resident/tenant’s family member.

  • It is always recommended that you get involved in the care of your senior. You can participate in care conferences and are encouraged to ask questions at any time. Whenever you see a change in your senior or are concerned about an issue, please immediately report it to the nurse as you know the senior the best. When information is shared with professional staff, better communication results and solutions are easier to find when everyone works together.

  • Your senior is encouraged to dress daiy. Consider having 5-7 outfits marked with the senior’s name available to wear. Comfortable slacks, long shorts, or sweat suits and loose fitting tops are appropriate. Supportive rubber soled shoes can assist in the prevention of falls and are strongly encouraged. A coat or jacket should be available. Chautauqua Guest Homes can launder all personal items or family are welcome to do this if they prefer.

  • We want your senior at ease in the new surroundings. Personal items such as photo albums or photos, a pillow, a bedspread, or wall hangings are a good start. Please remember to not bring expensive items or things with high sentimental value as these can be misplaced or broken. We strongly suggest that expensive jewelry not be brought to the facility as we do not have control over all visitors. A locked drawer is available in each room but it is preferred that valuables not be brought to the facility.

  • In most cases, an outing or leave of absence from Chautauqua Guest Homes is permissible with the approval of the attending physician. It is always best to communicate plans with facility staff to assure the senior’ needs are met when absent from the facility.
